LEVELS OF CARE
- The Parc offers 3 Levels of Care to adults needing assistance in activities of daily living “ADL”.
- The services included in each Level of Care are in addition to the Amenities & Services available to all residents.
- The appropriate Level of Care for each resident is assessed prior to move-in and reviewed throughout residency.
- Generally speaking, Levels of Care are determined by the amount of Staff time and involvement required. Prospective residents may not need all the services listed in Level of Care but still fall into a Level because they need certain services. In addition, Levels of Care change over time, assessments are periodically reviewed with the family.
Level 1 Services:
- Nurse oversight
- Daily reminders (Meals, activities, etc.)
- Stand-by assistance with ADLs (Resident performs most of the task with limited Staff assistance)
Level 2 Services:
- Nurse oversight
- Fall risk; history of falls
- Medication management
- Moderate assistance with ADLs (Staff performs more of the task than Resident)
- 2 hour checks (7:00 p.m. – 7:00 a.m.)
- Escort to meals and activities
Level 3 Services:
- Nurse oversight
- Fall risk; history of falls
- Extensive to total assistance with AOL’s (2 person assistance)
- 2 hour checks (7:00 p.m. – 7:00 a.m.)
- Incontinence
- Required medical devices (catheter, colostomy bag, bi-pap, c-pap, etc.)
- Cognitive diagnosis
- Medication management
- Additional laundry as needed
